Gifts in memory or gifts in honor
If you would like to make a gift in memory or in honor of someone, please check off “dedicate my gift” in the donation form below. If you would like us to mail a notecard notifying an individual that you made a memorial gift or notifying an individual that you made a gift in their honor, please fill out all the information on the form including name of person you would like the notecard sent to and full mailing address. Note: donation amounts are not included in notification cards unless by specific request. Contact btarallo@csac-vt.org with questions.

Amy’s Fund
A gift to Amy’s Fund brings joy-filled opportunities, connection and community to residents of CSAC’s five developmental services group homes. Smiles abound thanks to Amy’s Fund – residents have enjoyed trips to Vermont State Parks, concerts, movie outings, weekly bowling, trips to Winter Lights, Field Days and more. To make a gift to Amy’s Fund, please choose “Amy’s Fund” in the drop-down menu of the donation form below. To donate by check, please make your check out to “CSAC” (include “Amy’s Fund” in the memo line or include a note stating you’d like your gift to support Amy’s Fund). Barbara Hammerlind & Ric Wheeler Memorial Bench
In memory of Barbara Hammerlind and Ric Wheeler, and with the blessing of their families, we invite you to support installation of memorial bench at our 89 Main Street location. Barbara, an Advanced Practice Registered Nurse served over 41 years in CSAC’s Adult Mental Health Services. Ric spent 19 years as Manager of Supported Employment. Together they touched many lives, both through community involvement and their work at CSAC. To support the Memorial Bench, please choose “Barbara Hammerlind and Ric Wheeler Memorial Bench” in the drop-down menu of the donation form below. Or make check payable to “CSAC” (include “Barbara & Ric Memorial Bench” in the memo line or a note stating your gift supports the Memorial Bench). Mail your check to: CSAC, Attn: Development 89 Main St, Middlebury VT 05753. Any funds raised in excess of the Memorial Bench project will support CSAC’s Emergency and Crisis services.
