Children’s Developmental Services

Navigating childhood supports can feel overwhelming, but you do not have to do it alone. CSAC’s Developmental Services division partners directly with youth and families to help you access, manage, and coordinate the state-funded resources your child may be eligible to receive.

Core Support & Funding Programs

If your child has a diagnosed developmental disability and is eligible for Vermont Medicaid, several programs are available to help maintain health, safety, and community inclusion.

CSAC’s Bridge Program: Care Coordination

What it is: Dedicated, one-on-one navigation support to act as your advocate and anchor.

What it does: Your CSAC Care Coordinator helps you build an individualized care plan, assists with complex applications, and coordinates care across schools (including IEP/504 planning), medical teams, and community resources.

Eligibility Age: Up to participant’s 22nd birthday.

Family Managed Respite (FMR)

What it is: Funding designed to give primary family caregivers a necessary, planned break from ongoing daily care.

What it does: It can be used regularly or during a crisis to provide a break for you and a fun, different routine for your child.

Eligibility Age: Up to participant’s 22nd birthday.

Flexible Family Funding (FFF)

What it is: Highly flexible, income-based grant allocations determined by a sliding scale. Maximum of $1000 annually.

What it does: These funds are used strictly at the discretion of the family to pay for any legal good or activity that supports the child, including home modifications, assistive technology, individual household needs, or recreational activities.

Children’s Personal Care Services (CPCS)

What it is: A family-directed Medicaid program overseen in partnership with the Vermont Department of Health.

What it does: Provides financial assistance to pay for one-on-one support workers to help your child complete age-appropriate daily care routines, including bathing, dressing, grooming, eating, toileting, or mobility.

Eligibility Age: Up to participant’s 22nd birthday.

How a CSAC Care Coordinator (available through Bridge) Helps Your Family

When you enroll in services, a dedicated coordinator works directly alongside you to handle the heavy lifting:

  • System Navigation: Teaches you program rules, allowable expenditures, and budgeting so you can make informed choices.
  • Transition Planning: Provides specialized roadmap planning as your teenager approaches adulthood, helping them transition seamlessly into adult developmental services and vocational paths.
  • Collaboration & Advocacy: Stands with your family during school meetings, medical reviews, or periods of crisis to ensure your child’s voice and needs are prioritized.

Getting Started With Community Associates

CSAC is a regional designated agency that administers this program through it’s developmental services (DS) division called Community Associates. Initial eligibility for all DS programs is completed by Public Consulting Group (PCG) which contracts with the VT Department of Aging and Independent Living.

If you need assistance connecting with PCG or would like to speak with someone about these services, please contact our Bridge & Children’s Services Care Coordinator at Community Associates.  (802) 388-4021
