Community Associates is authorized to provide Adult Family Care Services. This is a Choices for Care program for person’s who do not have a developmental disability, meet the criteria for nursing home level care, but do not want to live in a nursing home. Services include an individually matched home placement, the potential of adult day services ( Project Independence) if funded, and Service Coordination.
