Community Associates also provides Traumatic Brain Injury services & Adult Family Care homes (through the Choices for Care program)
Conflict-Free Case Management means that the people who help plan your care (case managers) must be separate from the people or organizations that provide your care. This ensures that your care is planned fairly and that your needs come first.
In Vermont, people working for organizations that provide HCBS services cannot have a financial connection to the organizations that offer case management.
These rules ensure that your case manager focuses only on what is best for you and your care.
